Pay your bill

You can easily pay your bill online. Before making your selection, please check your billing statement for the correct service provider.

Health insurance

If we participate with your insurance plan, we will submit your claim. If we do not, as a courtesy, we will submit the claim on your behalf and you will continue to get a statement from us until payment is made.

Managed-care plans

We participate with many managed care plans. The key to smooth processing of these claims is the Referral Form from your Primary Care Physician. Under these plans, to see us for an office visit, you must provide a Referral Form or sign a Statement of Personal Responsibility. Co-pays, if required, are expected at the time of service.

Worker’s Compensation & NYS No Fault Insurance

For employment or automobile accident injuries, we will file your Worker’s Compensation or No-Fault Insurance claim. Just bring the required information (our staff can assist you) to your appointment. You and your employer’s cooperation will help assure timely submission of the claim.

Medicare

We welcome Medicare patients and are pleased to submit your Medicare and any secondary insurance claims.

Please note that our policy requires payment in full at the time of treatment. If you are not covered by an insurance plan with which we participate, please understand that you are ultimately responsible for payment.
